- COVID-19 Notice
In order to minimize the exchange of papers and materials, we now require that you email your:
- Insurance cards and/or information
- Referring dentist’s treatment referral form
- Any appropriate x-rays.
These must be sent to our office prior to scheduling your consultation appointment
